    slide hammer puller, with a wheel stud bolt welded on the tip of the tool has worked very well for me, in addition if you have some space, i use hydraulic bottle jack (little 2 tons) inside (between frame rails channel, caution not to bend!) and i put a pressure on the axle while im slide hammering !
