In my experience, the shifter pops out of gear for a couple reasons:
It was not positioned properly in gear to begin with. There is a spring and Ball that fits in a notch in your shaft. If the gear is not positioned correctly, the ball does not sit in the notch and the gear is free to walk away.
Gear Faces are worn with a slight crown. This promotes the "Walk Away" effect of the gears. Usually caused by not getting the shifter positioned properly to begin with, and hets worse over time.

Some guys have been building "Shift Clamps" to hold their shifter in place, not sure how well it works for them.
